I voluntarily consent to submit to become a participant in the validation phase of Red Moon Recovery LLC’s (“Red Moon Recovery Menstrual Cycle Mapping app development (the “APP”). I further authorize and give full permission to have Red Moon Recovery and/or its authorized agents to receive my data and related feedback, subject to the Data Fidelity Statement and Privacy Policy.I agree to release and hold harmless Red Moon Recovery and its agents from any liability arising in whole or part, during the validation phase of the APP and use of the information from said test phase. I further agree that a reproduced copy of this consent and release form shall have the same force and effect as the original.This consent and release has been explained to me in a language that I understand, and any questions that I had about this consent and release have been answered.
